Performance review checklist for the week when you need your proof in one place

A review goes sideways when you try to remember six months of work from memory. Use this checklist to gather proof, write sharper bullets, and pick the paid help that fits the week you are in.

Use this page when: the review is real, your notes are messy, and you need a plain order of operations.

The checklist

  1. Open your calendar, Slack, Jira, email, or notes from the last 90 days.
  2. Pull out three to six pieces of work that changed time, money, quality, risk, or team load.
  3. Write one line for what was messy before you stepped in.
  4. Write one line for what you did, in plain language.
  5. Add one proof point: a number, a quote, a shipped link, a customer note, or a before-and-after detail.
  6. Group the strongest items by theme, such as delivery, leadership, customer impact, or process improvement.
  7. End with the next ask: strong rating, bigger scope, promotion, or a later pay conversation.

I helped with the release and kept things moving.

Better: I pulled the release blockers into one tracker, set owners and dates, and cut the late Friday chase-up because the team could see the same record.

Weak note

I improved communication across teams.

Better: I replaced ad hoc updates with one written handoff note, which cut repeat questions and gave support the same answer path every time.

Weak note

I took on more ownership this quarter.

Better: I moved from doing assigned work to shaping the handoff, priorities, and review loop, which changed the pace of delivery for the rest of the team.

If the review is tomorrow

Do not try to build the perfect master document. Pick the top three pieces of proof, write the before, action, and result for each, then move into a cleaner template.

Use the 40 minute review plan if the deadline is immediate. If the review is tied to a warning or recovery plan, use the PIP response template first.

Pick the next step by what is missing
